This screening disclosure form is a Swim Alberta requirement. Members are welcome to check out Swim Alberta's website for the specific details on their policies, including their Screening Policy, found under Safe Sport on their Governance webpage (Swim Alberta's website). All members of Edmonton Keyano are subject to the Participation Points Policy and all Keyano Safe Sport policies.
